In order to begin the development of the client's Life Care Plan, we must request the following information:

• A medical release form signed by the client
• All medical records, reports, and bills associated with the injury
• Name, address, phone number, e-mail of the client
• A signed copy of our standard contract for services
• The date by which the Life Care Plan should be provided
